Consensus and Future Directions on the Definition of High On-Treatment Platelet Reactivity to Adenosine Diphosphate
Abstract: The addition of clopidogrel to aspirin treatment reduces ischemic events in a wide range of patients with cardiovascular disease. However, recurrent ischemic event occurrence during dual antiplatelet therapy, including stent thrombosis, remains a major concern. Platelet function measurements during clopidogrel treatment demonstrated a variable and overall modest level of P2Y(12) inhibition. High on-treatment platelet reactivity to adenosine diphosphate (ADP) was observed in selected patients. Multiple studies …

“…Our findings are not in agreement with the results of a recent metaanalysis [2], which reported a significantly higher risk of major adverse cardiovascular events in carriers of even one CYP2C19 LOF allele. The discrepancy could be attributed to the fact that the effect of one CYP2C19 LOF allele on the pharmacodynamic response of clopidogrel is less pronounced than two LOF alleles in Chinese patients.…”

“…We performed a subgroup analysis on different platelet function assays and found that LTA and MEA-guided IATs improved the combined endpoints of the CLR patients, whereas VASP- and VerifyNow-guided IAT did not. These results are consistent with reports that LTA and MEA are highly correlated to ischemic events[ 27 28 29 30 ] although Tang et al . [ 20 ] found that TEG-guided IAT also decreased the incidence of ischemic CV events compared to CAT.…”

“…This is in accordance with previously published studies applying similar platelet function testing [12], [44]. We found that in clopidogrel responders, additional serotonin dose-dependently increased ADP-induced aggregation, i.e.…”
